License Server Change Request
hereby requests a change of the license server for FLOW-3D products (“the Software”) from the Customer computer where the license manager is currently installed (designated below under “From Computer ID”) to another Customer computer (designated below under “To Computer ID”). In connection with this request, Customer hereby acknowledges and agrees as follows:

Flow Science has no obligation to deliver new license files in connection with this request. At its discretion, Flow Science may deliver a new license file to facilitate the requested server change and Customer shall promptly complete the change as permitted herein. Immediately upon installation of the license file on the new computer, Customer shall permanently destroy and erase the license file previously issued to Customer by Flow Science for the same Software and shall ensure that the Software is no longer accessed or used in connection with prior license files or the “From Computer ID”. Upon Flow Science’s request, Customer shall certify in writing that it has complied with its obligations under this Form. Nothing herein shall increase the number or scope of the license(s) granted under the governing license agreement, and failure of Customer to timely destroy and erase the license file issued to Customer by Flow Science shall result in termination of the Software license granted to Customer.

Determining MAC Address: Windows – Open a Command Prompt, type "ipconfig /all" and provide the Ethernet adaptor's physical address. Linux – Open a terminal window, type "ifconfig -a" and provide the HWaddr.
Determining Host Name: Windows – Click Start button, right-click the Computer icon. Select Properties. The “Computer name” is the Host Name. Linux – Open a terminal window, type “uname -n”. Host Name is returned.
